CareBridge was founded because the Australian healthcare system is built for acute illness — not the long, exhausting diagnostic journey that complex chronic conditions require. I built the service I wish had existed when I needed it.

The story
When you have a complex condition, the Australian system moves in slow motion. Specialist waitlists stretch 12 months. GP appointments are 10 minutes. The referral letter that could change everything often never gets written at all — not because your GP doesn’t care, but because they don’t have time to research a condition they see once a year.
CareBridge was built to solve that specific problem. Not by replacing your healthcare team, but by doing the work that falls between appointments: researching your condition, mapping your symptoms to diagnostic criteria, presenting your case in clinical language, and identifying the right specialist pathway.
The result is a GP-ready document you can bring to your next appointment — so your 10 minutes are spent moving forward, not explaining from scratch.
The background
My name is Antonella Soarez Dornelles. I am a health navigator based in Perth, Western Australia. I completed medical school internationally and have published peer-reviewed research in a medical journal — which means I can read the literature, evaluate the evidence, and translate it into language a clinician will recognise and respect.
I am not registered with AHPRA or the Medical Board of Australia, and CareBridge is not a clinical practice. What I offer is something different: the capacity to sit with a patient’s story, locate it within the medical literature, and build the document that gets it taken seriously.
I also have a personal understanding of what it is to navigate chronic illness in the Australian healthcare system — the exhaustion, the scepticism, the relief when someone finally takes your symptoms seriously. That experience shapes how CareBridge works.

All report services are priced in advance. There are no billable hours, no surprise invoices, and no incentive to extend the work beyond what you actually need.
CareBridge prepares documents for your healthcare team — it never tries to replace them. Every recommendation defers to your treating clinicians.
All information shared with CareBridge is handled in strict confidence under the Australian Privacy Act 1988. Health information is never shared without your explicit written consent.
A 15% concession is available for Pensioner Concession Card holders and international students on a valid Australian student visa.
