A medically trained navigator personally reviews your case and organises your health narrative.

3

You receive a clear, GP-ready document with your history, evidence, and prepared questions.

4

Follow-Up Support

After your appointment, a complimentary check-in ensures you feel heard and clear on next steps.

Your Care Doesn't End at the Appointment.

After your GP appointment, I offer a complimentary follow-up check-in to reflect on how the consultation went — whether you felt heard, whether your questions were addressed, and whether you feel clearer about next steps.

If needed, we can update your summary or prepare for future discussions. This is part of my commitment to ensuring you feel supported throughout the process.

This remains an educational and reflection-based conversation only — all medical decisions continue to rest with your treating doctor.

🤝

Complimentary Follow-Up Included

Every case review includes a post-appointment check-in. No extra charge, no time pressure.

📝

Summary Updates

Revise your summary or prepare materials for future specialist referrals.

Frequently Asked Questions

Is CareBridge a medical service? Can you give me a diagnosis?
No. CareBridge is a health education and navigation service. We help you understand and organise your health information, and prepare you for conversations with your treating doctor. All medical decisions, diagnoses, and treatment plans remain with your registered GP or specialist. The Health Navigator holds an international medical degree but is not registered with AHPRA.
Who reviews my case?
Every case is personally reviewed by a medically trained navigator who holds a medical degree from the Universidad Nacional de Córdoba, Argentina, and clinical research experience in Australia. They are not registered with AHPRA and act as a health educator and navigator, not a clinician.
What documents should I bring or share?
Any documents you have — blood tests, imaging results, referral letters, discharge summaries, medication lists, or even handwritten notes. Even if it feels disorganised, that's exactly what we're here to help with.
How long does a case review take?
Tier 1 (GP Consult Ready) is delivered in 3–5 business days. Tier 2 (Health Evidence Summary) in 5–7 days. Tier 3 (Complex Case Navigation) in 7–10 days. We'll confirm your timeframe at booking.
Is my health information kept private?
Absolutely. All information is handled in strict confidence and in accordance with the Australian Privacy Act 1988 and the Australian Privacy Principles. Your information is never shared with third parties without your explicit written consent.
