CareBridge is most useful when a patient has a complex or under-documented history, is heading into a high-stakes specialist appointment, or has struggled to communicate their full symptom picture in short consultations.
When a patient has a referral in hand but a history that is difficult to summarise in a letter — ADHD, endometriosis, Long COVID, POTS, and similar conditions where context takes time. CareBridge prepares a document your letter can reference.
When a patient is being transitioned between services or is approaching a multi-specialist pathway and needs someone to hold the whole picture. CareBridge bridges the gap between appointment types.

Scope of service
CareBridge operates strictly within an education and navigation model. This is not a clinical service, and Antonella does not provide clinical advice to patients or clinicians.

About the founder
Antonella Soarez Dornelles holds a medical degree from Brazil and has worked in research settings at the University of Western Australia and Royal Perth Hospital, with a peer-reviewed publication in her field. She is not registered with AHPRA or the Medical Board of Australia and does not practise clinical medicine in Australia.
CareBridge is a health navigation and education service — Antonella's clinical background informs the quality of the preparation she provides, but all clinical decisions remain firmly with each patient's own treating clinicians.
CareBridge is available in English, Spanish, and Portuguese, and serves patients across Perth and regional WA, as well as other parts of Australia via telehealth.
