For GPs, nurses & allied health

A trusted resource for patients who need more time than you have.

CareBridge works alongside clinicians — not instead of them. When a patient needs help preparing for a specialist appointment, organising their history, or navigating a complex referral pathway, CareBridge provides structured, education-based support that makes your job easier.

Who refers to CareBridge

Clinicians who want their patients to arrive at specialist appointments prepared.

CareBridge is most useful when a patient has a complex or under-documented history, is heading into a high-stakes specialist appointment, or has struggled to communicate their full symptom picture in short consultations.

General Practitioners

When a patient has a referral in hand but a history that is difficult to summarise in a letter — ADHD, endometriosis, Long COVID, POTS, and similar conditions where context takes time. CareBridge prepares a document your letter can reference.

Practice nurses & care coordinators

When a patient is being transitioned between services or is approaching a multi-specialist pathway and needs someone to hold the whole picture. CareBridge bridges the gap between appointment types.

Allied health professionals

Physiotherapists, psychologists, occupational therapists and dietitians whose clients are navigating parallel medical pathways. CareBridge helps ensure medical appointments are as productive as allied health appointments.

Support workers & NDIS planners

When a participant needs help navigating medical appointments as part of their support plan. CareBridge provides evidence-based appointment preparation that complements NDIS-funded supports.

Social workers

When a client is dealing with both health complexity and social determinants — language barriers, health literacy, or system navigation challenges. CareBridge can support in English, Spanish, and Portuguese.

Specialists

If a patient arrives without an adequate history and you would benefit from having them return with organised documentation — CareBridge can prepare that document before their next appointment.
